Enrollment

In order to enroll in any of our trainings, you must complete the following 2 steps that are listed below. Due to California state laws we need a hard copy of your enrollment agreement and a signed School Performance fact sheet which are both attached below.

1. Fill out the ENROLLMENT FORM and sign the SCHOOL PERFORMANCE FACT SHEET (Click Here) and mail all to us at CCNM, 1237 South Victoria #169, Oxnard, CA 93035. If you are enrolling from another country or state, you may fax in your enrollment form in addition to mailing it.
2. Fill out the Enrollment Registration form below so that we know to expect your mailed forms..
