Enrollment Forms

In order to enroll in any of the courses, you must complete the following 2 steps that are listed below.

1. Fill out the online enrollment form below.
2. Fax in a copy of the enrollment form (Click Here)
3. Due to California State laws we need an original signature on file.  After faxing your enrollment form, mail us the original.
