Enrollment

In order to enroll in any of our programs, please complete the following 4 steps listed below. Due to California state laws, CCNM must obtain a hard copy of your enrollment agreement and signed School Performance fact sheet. These forms are attached below.

1. Complete the ENROLLMENT FORM [download here]
2. Sign the SCHOOL PERFORMANCE FACT SHEET (Go to the training and download school performance fact sheet and email and fax to the institution) Or

   2018 SPFS Homeopathic Medical Practitioner

   2018 SPFS Nutraceutical Consultant

   2018 SPFS Naturopathic Practitioner

   2018 SPFS Holistic Health Practitioner

3. Mail both the Enrollment Form and School Performance Fact Sheet to CCNM at:
